Case Management Assistant

Hourly Wage Estimate: $20.40 - $28.56 per hour. Job Summary and Qualifications The Case Management Assistant (CMA) works under the direction of the RN case manager or social worker to facilitate patient discharges that require post‑acute placement or services and to identify and report discharge barriers. The CMA provides administrative support in case management functions. Responsibilities Coordinate with RN case managers and social workers for necessary support. Provide clerical support in multidisciplinary rounds, including updating NATE Tempo as directed by the team. Make referrals through the web‑based referral system (nH Discharger/other) to the requested post‑acute care providers, sending the appropriate patient information from the medical record. Monitor Medicare patients’ planned discharge dates and deliver the Important Medicare Message (IMM) to the patient per facility policy. Qualifications Associate degree preferred. Medical knowledge and terminology required. Minimum one year of experience in a health‑care setting required.
