Key Responsibilities

• Direct and implement personalized physical therapy treatment plans to prevent disability, alleviate pain, and restore muscle function, closely adhering to each client's outlined care plan.
• Oversee and mentor physical therapy assistants to ensure adherence to the prescribed plan of care.
• Conduct comprehensive evaluations of clients' physical functioning, employing diagnostic tests to assess muscle, nerve, joint, and functional capabilities.
• Analyze and adapt rehabilitation strategies by evaluating clients' home environments and providing recommendations to foster greater independence.
• Successfully deliver on-site physical therapy services, timely documenting clinical records, patient evaluations, and progress notes in alignment with clinical guidelines.
• Administer various therapeutic modalities including manual therapy, therapeutic exercises, traction, and massage to optimize patient recovery.
• Evaluate, fit, and modify prosthetic and orthotic devices, making appropriate recommendations to physicians for any necessary adjustments.
• Collaborate with interdisciplinary teams, participating in case conferences to discuss and refine comprehensive treatment strategies for clients.
• Serve as a resource to clients and their families, providing education about therapy techniques, device usage, and overall rehabilitation processes.
• Maintain accurate records of patient progress and treatment responses, ensuring open communication with referring physicians and case managers regarding any alterations in the treatment plan.
