Key Responsibilities\n

As a Licensed Professional Counselor at Social And Family Consulting, your primary responsibilities will include:

• Conducting comprehensive assessments to identify the unique mental health needs of each patient.

• Facilitating effective communication among supervisors, direct care staff, family members, and guardians to ensure holistic care and progress tracking for clients.
\n
• Creating tentative schedules for appointments, home visits, and monitoring observations to ensure the ongoing effectiveness of therapeutic interventions.
\n
• Coordinating in-home visits and observations necessary for accurate assessments, chart reviews, and data collection.
\n
• Ensuring the proper execution and modification of treatment plans, responding to feedback from multidisciplinary team reviews.
\n
• Participating actively in Interdisciplinary Team Meetings to discuss client progress and align on best therapeutic practices.
\n
• Documenting monthly and quarterly progress notes to track patient developments and treatment outcomes.
\n
• Organizing and leading training sessions to equip staff with the necessary competencies for implementing program plans effectively.
\n
• Providing continued support through follow-up interviews and home visits to reinforce the client's treatment plan.
\n
• Maintaining up-to-date records that allow for transparency and responsiveness among all stakeholders involved in a patient’s care.

• Acting as a liaison with community partners and mental health organizations to foster collaboration and support for program development.

• Assisting in the creation and revision of training materials that enhance overall care programs for individuals.

Qualifications\n

To be successful in this role, candidates must possess the following qualifications:

• A Master’s or Doctoral degree in Counseling, Psychology, Social Work, or a related field.

• Active and unrestricted licensure as a Professional Counselor in the state of Kansas.

• A minimum of 2 years of professional counseling experience in a clinical or outpatient setting is required.

• In-depth knowledge of legal and ethical standards governing clinical practice.

• Strong interpersonal and communication skills, enabling the establishment of trust and rapport with a wide range of clients.

• A demonstrated capacity to work collaboratively within a healthcare team.

• Proficiency in Electronic Medical Records (EMR) software and related technological tools to facilitate seamless documentation and care delivery.

Preferred Experience\n

Candidates who have specialized experience in family therapy, couples therapy, trauma recovery, or substance misuses will be given priority. Compensation and Benefits\n
