Youth Case Manager — Community-Based Support

Riverwoodcenter

Riverwood Center is seeking a dedicated professional to provide community-based case management for children and adolescents with intellectual and developmental disabilities, including autism, and their families. The role involves planning, advocacy, coordination, monitoring, and helping clients access health services, financial assistance, employment, education and social supports.
