Patients per day- 18Procedures- standard urology procedures in the clinic and operating roomRequired clinical activity in the last 24 months case logs- Applicant must provide documentation case list of provision of clinical services representative of the scope and complexity of the privileges requested during the previous 24 months.Will provider work solo? If not other providers specialties working with them- YesLicensed Candidate: Yes candidate must have a Washington state license in hand at time of present.Required certifications Must be from American Heart Association or American Red Cross as applicable- BLS and ACLSEMR - EPIC Shift Info: Day 5x9-Hour 08:00 - 17:00
