Demographics – Average patients per day: 18. Procedures – Standard urology procedures in clinic and operating room. Required clinical activity in the last 24 months: applicants must provide documentation of a case log that represents the scope and complexity of the privileges requested during the previous 24 months. Will provider work solo? If not, other provider specialties working with them – Yes. Licensed candidate: Yes – must have a Washington state license in hand at time of present. Required certifications: Must be from the American Heart Association or American Red Cross as applicable – BLS and ACLS. EMR – EPIC. Shift info: Day, 5x9 hours (08:00 – 17:00).
