Key Responsibilities

• Conduct comprehensive evaluations that involve analyzing patient history, performing necessary assessments, and documenting findings and recommendations for treatment.
• Communicate evaluation results and treatment plans clearly to patients and their families while actively engaging them in goal setting, ensuring their understanding and participation.
• Implement and adjust treatment protocols based on your evaluations and the continual assessment of patient needs, utilizing evidence-based practices to improve therapeutic outcomes.
• Collaborate with other healthcare professionals to enhance patient care, recognizing alterations in a patient's condition that may require modifications in their care plan and pursuing necessary referrals.
• Manage discharge planning effectively, ensuring continuity of care with timely and precise documentation of patient interactions, treatment sessions, and progress notes.
• Demonstrate effective time management skills, maintaining productivity and punctuality while facilitating patient access to care.
• Contribute to the department's goals and initiatives while adhering to all regulatory and accreditation standards.
• Engage in ongoing professional development through continuing education opportunities and act as a clinical instructor for students.

Patient Population / Client Focus

Qualifications

• A Bachelor's Degree in Physical Therapy or a related field.
