Patient Care Coordinator
We are seeking a dedicated Patient Care Coordinator in the homecare industry. This role involves managing daily patient care and coordinating caregivers to ensure high-quality service and accurate recording of caregiver clock-ins and outs. Proficiency in programs like HHA Exchange and experience with Smartsheet's is advantageous. The ideal candidate is motivated, patient, and proactive.
Key Responsibilities:
- Oversee daily patient care activities and caregiver scheduling.
- Communicate effectively with patients, families, and caregivers.
- Adjust care plans as necessary.
Qualifications:
- Strong communication and problem-solving skills.
- Proficiency in English and Spanish.
