Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate the emotional, social, and financial needs of patients receiving hospice care.

• Collaborate with interdisciplinary medical teams to create comprehensive, patient-focused care plans that address the unique needs and situations of each patient.

• Provide individual and group counseling sessions aimed at helping patients manage feelings of grief, loss, and adjustment to their conditions.

• Develop post-discharge plans and facilitate connections with community organizations and resources to ensure continuity of care after hospice.

• Educate patients and families about available treatment options and emotional resources, empowering them to actively participate in their care.

• Engage in crisis intervention and counseling that addresses urgent emotional needs and provides immediate support during critical moments.

• Maintain accurate clinical documentation in compliance with hospital policies and state regulations, ensuring the integrity and privacy of patient records.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

Compensation and Benefits\n
