Key Responsibilities

• Assess and evaluate patients' physical conditions by conducting detailed examinations and utilizing clinical assessments to develop personalized treatment plans.
• Implement therapeutic interventions to enhance patient mobility and function, guiding an average of 7 patients through tailored exercise regimens and rehabilitation protocols each day.
• Monitor and document patient progress comprehensively, ensuring accurate records are maintained within our electronic health record (EHR) system to facilitate interdisciplinary collaboration.
• Provide education and resources to patients and their families, fostering an understanding of treatment goals, home exercise programs, and strategies for reducing pain and enhancing function.
• Work collaboratively with the care team, participating in regular multidisciplinary meetings to discuss patient progress and adjust treatment plans accordingly.
• Engage in community outreach and education initiatives that promote health and wellness, contributing to our facility's mission of providing holistic care.

Patient Population / Client Focus

Our rehabilitation center serves a diverse patient demographic, including individuals recovering from post-surgical procedures, orthopedic injuries, stroke, and other chronic conditions. Qualifications

• Valid New York State Physical Therapist License.
• Bachelor's degree in Physical Therapy; a Master's or Doctorate degree is strongly preferred.
• A minimum of two years of clinical experience in a rehabilitation or healthcare setting, demonstrating a track record of successful patient outcomes.

Preferred Experience

• Experience in a Skilled Nursing Facility or related environment.

Compensation and Benefits

At Excelera Physical Therapy, we value our team and offer a competitive salary ranging from $100,000 to $120,000 per year, commensurate with experience.
