Key Responsibilities\n

• Schedule and facilitate appointments, home visits, and therapeutic observations in order to assess the client’s evolving needs and treatment effectiveness.

• Participate actively in Interdisciplinary Team Meetings, providing insights from your counseling sessions to help shape holistic treatment approaches for clients.

• Document clinical observations and progress notes on a monthly and quarterly basis, ensuring that all information is accurate and accessible for future reference.

• Conduct follow-up interviews and home visits to provide ongoing support and adjust care plans as necessary to meet changing client needs.

• Train and certify team members on the implementation of treatment protocols and thorough documentation practices, enhancing team competency.

Qualifications\n

Compensation and Benefits\n

In recognition of your valuable contributions, we offer a competitive salary ranging from $90,000 to $110,000 annually, contingent on your experience and client caseload.
