Key Responsibilities\n

• Conducting ongoing assessments tailored to the specific needs of each patient, ensuring comprehensive and personalized care.

• Establishing effective communication with supervisors, direct care staff, family members, and other stakeholders to monitor patient progress and health outcomes.
\n
• Providing supervisors with a structured schedule of appointments, home visits, and observations to facilitate the monitoring of patient development.
\n
• Coordinating home visits for assessment and direct observation which includes chart reviews, staff interviews, and data analyses tailored to patient needs.
\n
• Ensuring that program plans are implemented effectively and meet established clinical objectives.
\n
• Participating in Interdisciplinary Team Meetings, contributing insights from assessments and observations to develop comprehensive treatment strategies.
\n
• Documenting patient progress through detailed monthly and quarterly notes to monitor improvements and make necessary adjustments to treatment plans.
\n
• Training and certifying staff and team members on the implementation of clinical documentation and program plans.
\n
• Performing follow-up interviews and maintaining regular home visits to ensure continuous support and alignment with patient needs.
\n
• Maintaining accurate and accessible records while acting as a liaison to the public and mental health community to foster collaborative relationships centered around patient achievement.

• Assisting in the development of training materials and protocols to continually improve patient care and outcomes.

The LPC will work with a diverse client base, including both adults and youth facing various mental health challenges. Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related mental health field.

• Active and unrestricted licensure as a Licensed Professional Counselor (LPC) in the state of Georgia.

• A minimum of two years of professional counseling experience, preferably within a clinical setting.

• Knowledge of legal and ethical standards in mental healthcare practice.

• Strong interpersonal skills with the ability to develop rapport with diverse clientele.

• Demonstrated capacity to collaborate effectively within healthcare teams.

• Familiarity with Electronic Medical Records (EMR) software and relevant clinical technologies.
