Key Responsibilities\n

• Conduct comprehensive assessments to identify the specific needs of each patient, ensuring individualized care plans are in place.

• Maintain open communication with supervisors, direct care staff, and families to discuss patient progress and care strategies.

• Develop a weekly schedule for appointments, home visits, and follow-ups to ensure effective monitoring and support.
\n
• Coordinate home visits to conduct observations and interviews, collect data, and perform chart reviews that inform patient care decisions.

• Actively participate in Interdisciplinary Team Meetings to collaborate on care strategies and program effectiveness.
\n
• Document and prepare monthly and quarterly progress notes that reflect patient advancements and care adjustments.
\n
• Lead training sessions to educate staff and ensure competence in implementing program plans and documentation practices.
\n
• Provide continual follow-up through home visits and interviews to address the evolving needs of patients.
\n
• Ensure that all patient records are accurately maintained and readily available for review by authorized parties.

• Serve as a liaison with community partners and mental health resources to further develop service goals and patient outcomes.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Professional Counselor valid in Georgia.

• A minimum of 2 years of experience in a counseling role, ideally in a clinical setting.

• Proficient knowledge of legal and ethical counseling standards and practices.

• Strong interpersonal communication skills, with the capacity to establish rapport with clients from diverse backgrounds.

• Experience with Electronic Medical Record (EMR) software is preferred.
