Key Responsibilities\n

• Conduct comprehensive assessments of clients to develop individualized treatment plans that align with their unique needs.

• Maintain open communication with supervisors, caregivers, and family members about clients' progress, facilitating a supportive network for successful outcomes.

• Schedule and coordinate appointments and home visits, ensuring clients receive consistent follow-up and ongoing support throughout their healing journey.

• Implement program plans effectively, actively participating in team meetings to discuss strategies and collaborate on care plans.

• Document and maintain accurate records and progress notes on a monthly and quarterly basis, ensuring compliance with legal and ethical standards.

• Train and certify staff on program implementation, promoting a culture of continuous learning and development within the team.

• Serve as a liaison with the broader mental health community, advocating for clients and identifying opportunities for collaboration with other service providers.

Patient Population / Client Focus\n

You will engage with a varied patient demographic, including adults and youth facing challenges such as anxiety, depression, trauma, and relationship issues. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in the state of Illinois.

• A minimum of two years of counseling experience in a clinical setting is required.

• Strong understanding of legal and ethical standards in mental health counseling.

• Exceptional communication and interpersonal skills, enabling the establishment of rapport with diverse client populations.

Compensation and Benefits\n
