Key Responsibilities\n

• Conduct comprehensive assessments for clients to gauge their mental health needs accurately and develop personalized treatment plans.

• Maintain open lines of communication with supervisors, direct care staff members, as well as clients' families and guardians, to provide updates on client progress.

• Provide a detailed schedule to supervisors outlining appointments, home visits, and observations necessary for effective treatment tracking.
\n
• Coordinate and execute home visits to perform in-home observations, review patient charts, interview staff, and analyze relevant data.
\n
• Ensure the effective execution of program plans by actively monitoring treatment implementation and modifying approaches as needed based on client feedback.
\n
• Participate in interdisciplinary team meetings to discuss patient cases and develop collaborative strategies for client care.
\n
• Document and complete monthly and quarterly progress notes indicating clients' achievements relative to their treatment goals.
\n
• Organize and lead training sessions for staff to ensure competency in program plan execution and documentation standards.
\n
• Conduct follow-up interviews and home visits to provide ongoing support as clients progress through their therapy journeys.
\n
• Act as a liaison between clients and the community mental health resources, promoting awareness and understanding for better support services.

• Contribute to procedure development for training materials aimed at continuously improving the effectiveness of our programs.

Patient Population / Client Focus\n

As an LPC at Open Doors Counseling, you will work with a diverse range of clients, including both adults and youth, dealing with various mental health challenges. Qualifications\n

• A Master's degree or Doctoral degree in Counseling, Psychology, or a related field is required.

• An active and unrestricted license as a Professional Counselor (LPC) in the state of Arizona is mandatory.
