Key Responsibilities\n

• Clinical Interventions: Provide supportive counseling to patients and families, assist with linkage and referrals to community resources, and facilitate family discussions. Aid families in navigating complex decisions, such as hospice and placement options, while fostering effective communication with healthcare providers.

• Triage and Case Management: Proactively identify high-risk situations and manage a diverse clinical caseload. Collaborate with case managers on discharge planning and coordinate necessary resources to ensure seamless transitions for patients.

• Legal and Ethical Standards: Adhere to strict legal and ethical standards in all assessments and interventions, ensuring compliance with hospital policies regarding social services.

• Documentation: Maintain accurate and timely records in accordance with departmental guidelines, including interdepartmental plans, education forms, and progress notes.

Qualifications\n

Compensation and Benefits\n

The annual base salary for this position ranges from $120,000 to $130,000 depending on experience and qualifications.
