Coverage Area: Lakeland / Polk County, FL

Compensation & Earning Potential

• Pay-per-visit model: $70+ per visit + Medicare differential pay  
• Average full-time earnings: $120,000+  
• Quarterly bonus opportunities

Your Role as a Home Health Physical Therapist

• Evaluate patients and develop individualized treatment plans 
• Provide therapeutic exercises and mobility training 
• Educate patients and caregivers on safety and recovery 
• Collaborate with physicians and interdisciplinary teams 
• Supervise Physical Therapist Assistants (PTAs) 
• Complete documentation using streamlined technology

Qualifications

• Active, unencumbered Florida Physical Therapist license
