Work Environment

This role is primarily structured around home care, allowing you to engage with patients in the comfort of their own environment. Your flexible work schedule will take place Monday through Friday, affording you the work-life balance you need while meeting the rehabilitation needs of the community. You will work closely with patients, their family members, and an interdisciplinary team of healthcare professionals to create personalized care plans and treatment interventions.

Key Responsibilities

• Conduct thorough evaluations of patients' health histories, performing specific assessments to establish a baseline for developing individualized treatment plans.
• Clearly communicate the outcomes of assessments and proposed treatment strategies to patients and their families, ensuring all parties are involved in the goal-setting process to enhance patient engagement.
• Implement evidence-based treatment protocols tailored to patients' specific conditions while adapting to changes in their status during the course of therapy.
• Consult with other healthcare providers when necessary, documenting any notable changes in patients' conditions that may necessitate adjustments to the plan of care.
• Effectively discharge patients, creating comprehensive follow-up care plans and meticulously documenting every aspect of their treatment journey in compliance with healthcare regulations.
• Manage your time efficiently to meet productivity standards while keeping an accurate schedule of appointments and meetings.
• Actively participate in community outreach and marketing initiatives to promote the physical therapy services offered by Downtown Area Physical Therapy.

Qualifications
