Work Environment

This full-time position requires a commitment to onsite services within a home care setting, allowing you to work directly with patients in their homes. You can expect to work Monday through Friday with flexible shift options available, including 8 AM to 4 PM or 9 AM to 5 PM, as well as the possibility of four 10-hour shifts per week. Key Responsibilities

• Conduct comprehensive patient evaluations to identify movement impairments and functional limitations.
• Develop and implement individualized therapy plans that reflect each patient's specific goals and needs.
• Collaborate with a team of healthcare professionals, including physicians and occupational therapists, to enhance patient progress and recovery outcomes.
• Educate patients and caregivers on effective therapeutic exercises, self-care strategies, and preventative methods to promote health and wellness.
• Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
• Document patient treatment plans, evaluations, and progress notes in accordance with regulatory and company standards to maintain compliance.
• Participate in departmental meetings to share knowledge and discuss innovative treatment methodologies and patient-centric approaches.

Patient Population / Client Focus

You will work closely with a diverse patient population, including individuals recovering from surgery, injury, or chronic conditions such as arthritis. Our clients range from post-operative orthopedic patients to those requiring rehabilitation for neurological conditions. Qualifications

• A Bachelor's Degree in Physical Therapy is required.
• A valid Florida Physical Therapist License or eligibility is essential to practice within the state.
• Two years of clinical experience in physical therapy, particularly in outpatient or home care settings, preferred.
• Strong interpersonal and communication skills to develop rapport with patients and collaborate effectively with a multidisciplinary team.
• Demonstrated problem-solving skills and the ability to adapt treatment methodologies based on patient needs.

Preferred Experience

While two years of experience is required, candidates with additional qualifications, such as a Master's Degree in Physical Therapy, specialized certifications (e.g., orthopedic, neurological), or experience in a home care setting, will be looked upon favorably. Experience with electronic health records (EHR) systems is also a plus, enhancing documentation efficiency and patient tracking.
