Work Environment

As a Physical Therapist at Lakeland Care Solution, you will be working onsite in a home care setting. This allows for a flexible and intimate approach to patient care, enabling you to deliver therapeutic interventions tailored to each patient's environment. Your work week will typically consist of a Monday to Friday schedule, encompassing 40 hours per week. This role allows for a direct impact on the lives of patients who may benefit greatly from the convenience and familiarity of receiving care at home.

Key Responsibilities

• Conduct thorough evaluations of patients referred by physicians, ensuring a comprehensive understanding of their medical history and physical condition.
• Develop and implement personalized treatment plans that address a range of conditions, focusing on orthopedic, pediatric, neurologic, and sports medicine specialties.
• Utilize evidence-based practices to deliver skilled, hands-on therapeutic care, employing various techniques to improve patients' physical function and overall quality of life.
• Educate and train patients and their caregivers regarding treatment protocols, expected outcomes, and strategies for promoting recovery and avoiding further injury.
• Maintain accurate and timely documentation of patient progress and treatment outcomes, adhering to organizational policies and best practices.
• Collaborate with a multidisciplinary team to optimize patient outcomes,ensuring a holistic approach to each patient's care.
• Meet minimum utilization goals and contribute to business objectives, actively participating in initiatives that enhance organizational effectiveness.

Patient Population / Client Focus

You will have the opportunity to work with a diverse patient population, including individuals recovering from surgeries, sports injuries, neurological disorders, and more. Your role will involve closely monitoring their progress and adjusting treatment plans based on their unique needs and recovery goals.

Qualifications

• Bachelor's degree in Physical Therapy; candidates with a Doctorate of Physical Therapy (DPT) are preferred.
• Active state license to practice as a Physical Therapist in Florida.
• Current Basic Life Support Certification from the American Heart Association.
• Demonstrated ability to establish and maintain strong therapeutic relationships with patients and their families.
• Exceptional written and verbal communication skills, essential for effective patient education and documentation.
• Proficient in word processing and computer skills, necessary for managing patient records and care plans.

Preferred Experience

• A minimum of 2 years of relevant clinical experience in a physical therapy setting.
• Prior experience in home health care is highly desirable.
• A proven track record of utilizing research-driven treatments that yield positive patient outcomes.

Compensation and Benefits
