Key Responsibilities

• Conduct comprehensive evaluations by gathering pertinent medical histories, performing targeted assessments, and documenting results accurately.
• Communicate evaluation findings and individualized treatment plans clearly to patients and their families, ensuring collaborative goal setting that respects patient preferences.
• Demonstrate expertise in applying treatment protocols tailored to each patient's needs, adapting interventions as necessary based on their progress and evolving healthcare guidelines.
• Engage in interdisciplinary consultations when required, promptly adjusting treatment plans in response to any changes in a patient's condition.
• Facilitate seamless discharge planning, ensuring proper follow-up care and documentation that reflects the continuum of the patient's treatment journey.
• Effectively manage time, maintain a productive schedule, and actively participate in marketing and outreach efforts that enhance community engagement.

Qualifications
