Brooklyn Community Services in Brooklyn, NY is seeking a Community Health Worker - Street Outreach to support unhoused individuals at high risk for HIV and Hepatitis C. The role uses our Shower Bus and Mobile Outreach Van to deliver education, testing, counseling, and essential services.
