Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate clients’ mental health concerns, strengths, and support systems, ensuring all care starts with a personalized understanding.

• Provide individual, family, and group therapy utilizing evidence-based treatment modalities, including cognitive-behavioral therapy, trauma-focused therapy, and solution-focused therapy.

• Develop and implement individualized treatment plans outlining specific goals and interventions, regularly reviewing and adjusting these plans based on clients’ progress and feedback.

• Collaborate with psychiatrists and other healthcare professionals to ensure seamless client-centered care, discussing case management and treatment plans in team meetings.
\n
• Offer crisis intervention and risk assessment, employing safety planning as part of an immediate response to emerging client needs.
\n
• Maintain meticulous and timely documentation in alignment with ethical and legal standards, using our electronic health record (EHR) system for integrated client information management.

• Facilitate therapeutic group sessions, psychoeducation classes, and support groups to foster community connection and shared learning among participants.

Patient Population / Client Focus\n

In this role, you will work primarily with hospice clients and their families, addressing an array of mental health challenges that may arise throughout the end-of-life process. Your work will provide crucial emotional support during a sensitive period, helping clients process grief, anxiety, and other related issues while honoring their diverse backgrounds and values.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of experience in a mental health or outpatient setting, with familiarity in working with hospice clients being highly beneficial.

• Expertise in evidence-based therapeutic modalities and demonstrated understanding of various mental health treatment approaches.

• Exceptional interpersonal, communication, and assessment skills, enabling effective client engagement and relationship building.

Compensation and Benefits\n
