Title Registered Nurse - Behavioral Health (CTI Team) Location Bronx, NY (Onsite) Employment Type Full-Time – Monday to Friday. 09:00am‑05:00pm or 08:00am‑04:00pm Compensation Pay range: $90,000–$92,000 Per Year. Additional $2,000 for bilingual Spanish speakers. As the CTI RN, you'll provide community‑based medical and health support, working with a multidisciplinary team to bridge gaps in care and connect participants to essential services. Key Responsibilities Assess and monitor participants' physical, cognitive, and psychosocial health. Support transitions from hospital/ED to community with care planning and follow‑up. Provide health education, medication reconciliation, and disease prevention guidance. Link participants to community resources and healthcare providers. Collaborate with clinicians, care managers, and peer specialists to ensure continuity of care. Conduct field visits and provide flexible, community‑based support. Qualifications NYS RN license and registration required. Minimum 1 year of post‑licensure nursing experience preferred, ideally with behavioral health populations or community/home visits background. Strong knowledge of health promotion, disease prevention, and community resources. Ability to work flexibly in field‑based settings.
