Job Description

Key Responsibilities

• Provide nursing care, administer medications and treatments as prescribed.

• Conduct routine assessments and monitor health conditions.

• Accurately complete and maintain documentation, MARs, progress notes, and reports.

• Support individuals in participating in daily routines and community activities.

• Communicate health updates with families, team members, and medical providers.

• Provide training and mentorship to Direct Support Professionals on health-related supports.

• Promote dignity, respect, and self-direction for every individual.

Requirements

• Active LPN license in the state of Minnesota.

• Current CPR and First Aid certification.

• Valid Minnesota driver’s license with acceptable driving record.

• Must pass a MN DHS background study.

• Physical ability to lift and transfer up to 60 pounds occasionally.

• Experience with individuals with developmental disabilities preferred.
