Key Responsibilities\n

Your responsibilities as an LICSW at Love Bound Therapy will include:

• Conducting comprehensive assessments to evaluate the emotional, mental, and social needs of senior clients.

• Developing individualized care plans with clients and their families, aligning support services with their goals and preferences.
\n
• Providing counseling and emotional support to help seniors navigate transitions, such as moving into assisted living or dealing with significant losses.

• Championing clients' rights and advocating for access to vital community resources, ensuring that they receive the help they need.
\n
• Working collaboratively with nurses, caregivers, and other healthcare professionals to create an integrated approach to client care.
\n
• Conducting routine follow-up visits, either virtually or via home visits, to monitor and assess the effectiveness of implemented plans.

• Facilitating communication among family members involved in a client’s care, especially to address conflicts or differing perspectives on care options.
\n
• Providing education on community resources and support groups to enhance seniors’ social engagement and reduce feelings of isolation.

• Maintaining accurate and current documentation of client assessments, care interventions, and progress, using electronic health records (EHR) systems.

• Staying abreast of best practices in social work and current regulations, ensuring all actions align with ethical standards.

Patient Population / Client Focus\n

The primary focus of this role is on seniors—an often underserved population in our community. You will work one-on-one with clients, addressing a spectrum of psychosocial challenges including mental health issues, social isolation, and family dynamics. Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in Massachusetts.

Compensation and Benefits\n

The salary range for this position is competitive, between USD 60,000 and 100,000 annually, based on experience and qualifications. We offer a comprehensive benefits package that includes:
