Key Responsibilities\n

• Conduct comprehensive assessments and evaluations of clients' mental health needs and social determinants, formulating individualized treatment goals tailored to each client.

• Provide empathetic, one-on-one counseling sessions, addressing a variety of mental health and psychosocial issues, including but not limited to anxiety, depression, grief, and adjustment disorders.

• Lead and facilitate engaging group therapy sessions that promote community support, education on mental health topics, and sharing of experiences among participants.
\n
• Develop, implement, and regularly update personalized treatment plans incorporating measurable goals and evidence-based interventions to track clients’ progress.
\n
• Deliver crisis intervention services as needed, including conducting risk assessments, creating safety plans, and coordinating emergency resources for clients in acute distress.

• Advocate for clients’ rights and facilitate access to services, assisting them in navigating complex healthcare and community resources.
\n
• Maintain accurate and confidential documentation of client records, adhering to ethical and legal standards, including assessments, treatment plans, progress notes, and outcome evaluations.

Patient Population / Client Focus\n

Your work will primarily focus on clients receiving hospice care, their families, and other individuals facing complex social and emotional challenges related to end-of-life issues. You will play a critical role in providing support for grief and loss, helping families navigate the emotional landscape during times of significant change and distress.

Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited institution is required.

Compensation and Benefits\n

This position offers a competitive salary ranging from USD 80,000 to 95,000 annually, commensurate with experience.
