Key Responsibilities\n

As a Clinical Social Worker, your day-to-day activities will include:

• Designing and implementing individualized support packages that address the unique needs and circumstances of clients, ensuring every treatment plan is tailored to foster progress.

• Conducting comprehensive assessments through interviewing clients to gain insights into their current psychological state, strengths, and areas for improvement.

• Monitoring clients’ progress regularly and making necessary adjustments to treatment plans to reflect their evolving needs and experiences.

• Facilitating therapy sessions where you will provide guidance, support, and actionable advice to clients, empowering them to take steps towards recovery.

• Maintaining meticulous documentation of clients’ progress and changes in their treatment plans, ensuring compliance with regulatory requirements.

• Participating in case conferences and collaborating with multidisciplinary teams, sharing valuable insights and strategies for improved client outcomes.

Patient Population / Client Focus\n

This role primarily involves working with individuals facing emotional distress and life-threatening illnesses. Many of our clients at the Hospice Center require an empathetic approach that acknowledges the complexities of end-of-life issues. Qualifications\n

To be considered for this position, candidates must fulfill the following qualifications:

• A Master’s degree in Social Work (MSW) or a related field.

• Current LCSW licensure in the state of Texas.
\n
• Demonstrated expertise in social work with a minimum of 2 years of relevant experience.

Preferred Experience\n

While not mandatory, candidates with experience in hospice care or palliative services, as well as familiarity with grief counseling and trauma-informed care practices, will be given preference. Compensation and Benefits\n
