Social Worker II

Role Overview: The Social Worker facilitates patient care through resource coordination, ensuring optimal health outcomes, appropriate resource utilization, and respecting patient self-determination. Key Responsibilities Conducts complex psycho-social assessments and interventions, manages transition planning, and assists with cases involving adoption, abuse, and neglect. Leads multidisciplinary care conferences, develops and oversees transition plans, and communicates effectively with patients, families, and healthcare team members. Ensures accurate documentation and compliance with regulatory standards while providing education to stakeholders. Qualifications & Skills Requires a Master's of Social Work (MSW) and an active licensure or certification to practice. Preferred experience includes two years in acute hospital settings and an accreditation as an Accredited Case Manager (ACM). Additional Duties Supports post-acute care needs, facilitates referrals, and ensures care aligns with regulatory and accreditation standards.
