Key Responsibilities\n

• Conduct thorough assessments and evaluations to identify individual client needs, creating personalized treatment plans accordingly.

• Regularly communicate with supervisors, families, and relevant stakeholders to discuss client progress and modify treatment approaches as necessary.

• Maintain an organized and tentative schedule of appointments, home visits, and interventions to effectively track client advancements.
\n
• Coordinate and conduct home visits, ensuring compliance with in-home observational requirements while collecting data for analysis.

• Participate in Interdisciplinary Team Meetings to foster collaboration and share insights on individual client cases, contributing to comprehensive care.
\n
• Document detailed monthly and quarterly progress notes that reflect client outcomes and treatment effectiveness.
\n
• Train and certify other staff members in implementing treatment plans and documentation procedures, ensuring program integrity.
\n
• Act as a liaison between clients and the wider mental health community, working towards developmental goals and community outreach.

• Assist in devising training materials that improve clinical practices and enhance program efficacy for the individuals we serve.

Patient Population / Client Focus\n

This role primarily focuses on a diverse range of patient populations, including adults and youth experiencing various mental health challenges. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Licensed Professional Counselor (LPC) in the state of Colorado.

Compensation and Benefits\n
