Real Estate Specialist

Job Summary The Entitlement Specialist is responsible for ensuring that members maintain continuous Medicaid eligibility by overseeing the timely completion of Medicaid renewals, new Medicaid applications and RVI-3 conversions. The Specialist will assist members in gathering required documentation within designated timeframes to prevent lapses, support the establishment of Pooled Income Trusts, and perform other Medicaid-related functions, including leading the resolution of eligibility issues that arise. Essential Functions Maintain and update the list of members’ Medicaid eligibility dates. Conduct field visits to members’ homes to assist with eligibility tasks. Ensure completion of Medicaid recertifications 90–60 days prior to expiration. Assist members with establishing Pooled Income Trusts when necessary. Track all submitted Medicaid recertifications and address any issues with the supervisor. Coordinate with staff, members, family, and caregivers to obtain required documents within specified time frames. Lead resolution of difficult eligibility issues. Submit Medicaid Relinks to HRA and other LDSS offices. Maintain documentation and records of previously submitted Medicaid recertifications. Maintain relationships with the Office of Homecare Services and next of kin to facilitate the eligibility process. Forward initial Medicaid documents to the Medicaid unit at the Office of Homecare Services. Assist the ESH Intake Department with problematic Medicaid issues. Complete new Medicaid applications and RVI-3 conversions. Attend workshops and in‑services to enhance knowledge of changes in eligibility criteria. Participate in publishing the monthly Medicaid expiration list. Track progress of new applications, recertifications, and conversions submitted through the Epaces system. Monitor E-Paces to follow up on or identify potential eligibility issues. Assist the Director or designee of Social Services and the Finance department in resolving denied Medicaid claims. Assist in the removal of certain Medicaid restriction codes. Perform any other job‑related duties as requested. Education and Experience High School or GED required; Associate’s Degree preferred. Three (3) years of healthcare, insurance, or related industry experience, including at least two (2) years of enrollment/eligibility or similar experience required. Ability to deal with problems involving several concrete variables in standardized situations. Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Intermediate computer skills. Effective communication skills. Ability to work independently and within a team environment. Attention to detail. Familiarity with the healthcare field. Knowledge of Medicaid and Medicare. Critical listening and thinking skills. Proper grammar usage. Time management skills. Proper phone etiquette. Customer‑service orientation. Licensure and Certification Must have a valid driver’s license, vehicle, and verifiable insurance. Employment is conditional pending a successful driver’s license record check; if the result is unacceptable, the offer will be withdrawn or the employee will be terminated. Working Conditions This is a mobile position. Regular travel to various work locations—including homes, offices, and other public settings—is essential. Employees may be exposed to weather conditions typical of the location, and may be required to stand or sit for long periods. Residency in the assigned territory is required; exceptions may be considered for business need. Over 50% of the time may involve travel. Flexible hours, including the possibility of evenings and/or weekends, may be required to meet member needs. Compensation Range $47,400.00 – $76,000.00 (hourly/salary).
