Experienced Nurse Practitioner in Cardiology - Full-Time Role

Gotham Enterprises Ltd is seeking an exceptional Nurse Practitioner (NP) to join our dedicated team at our Assisted Living Facility located in Mastic Beach, New York. In this full-time, onsite role, you will play a vital part in providing high-quality healthcare within our outpatient practice of the Center for Advanced Cardiac Care (CACC). You will work under the guidance of our Chief Nurse Practitioner and the CACC Medical Director, contributing significantly to patient care and outcomes.

Key Responsibilities

As a Nurse Practitioner at Gotham Enterprises, your responsibilities will include:

• Obtaining detailed patient histories and performing comprehensive and targeted physical examinations according to best practices.
• Ordering and interpreting diagnostic tests to develop accurate diagnoses, managing chronic and acute patient issues proactively.
• Facilitating care for specific patient subsets, ensuring that a high level of autonomy and collaboration is maintained.
• Coordinating and communicating with family members, caregivers, and multidisciplinary teams to ensure seamless continuity of care.
• Documenting all patient interactions meticulously in our electronic medical record (EMR) system, ensuring compliance and accuracy.
• Participating in the formulation and adjustment of goal-oriented care plans that integrate health maintenance, anticipatory guidance, and therapeutic interventions.
• Engaging in patient education on disease management and preventative care strategies, enhancing patient understanding beyond clinical settings.
• Coordinating hospital admissions and collaborating with inpatient teams for safe patient transitions out of the hospital.
• Working closely with NP peers and the nursing team to triage daily patient inquiries, enhancing overall responsiveness to patient needs.
• Contributing to research protocols by integrating and disseminating findings into our clinical practices, supporting continuous improvement.

Patient Population / Client Focus

You will primarily focus on patients within specialized cardiac care, including those suffering from advanced heart failure, pulmonary arterial hypertension, and those post-heart transplant. The role requires a deep understanding of the complex needs of these patients, particularly those undergoing advanced therapies such as outpatient intravenous support or mechanical circulatory support, ensuring that individualized care is maintained.

Qualifications

The following qualifications are essential for this role:

• A Master’s degree in Nursing with specialization as a Nurse Practitioner, emphasizing adult care.
• Current licensure as a Registered Professional Nurse and Nurse Practitioner in New York State.
• NP Board certification showcasing competency in specialized practice areas.
• Current certifications in Basic Cardiac Life Support (BCLS), Advanced Cardiac Life Support (ACLS), DEA, and NPI.
• Proficiency with electronic medical records (EMRs) and various office productivity software, including MS Word, PowerPoint, and Excel.

Preferred Experience

While the minimum required experience is two years in a hospital or outpatient setting, candidates with expertise in cardiac care, particularly managing complex patient populations and advanced therapies, will be given priority. Your prior experience should ideally include working closely with multidisciplinary teams, reinforcing our collaborative care model.

Compensation and Benefits

The salary for this Nurse Practitioner position ranges from USD 120,000 to 220,000 annually, based on experience and qualifications.
