Full-Time Nurse Practitioner in Cardiology - Outpatient Setting

Gundersen Health and Wellness Clinic is excited to announce an opening for a full-time Nurse Practitioner (NP) to join our esteemed Center for Advanced Cardiac Care (CACC) located in Patchogue, New York. As an NP, you will report directly to the Chief Nurse Practitioner, collaborating closely with the CACC Medical Director and other healthcare professionals to enhance patient outcomes and ensure comprehensive care.

Work Environment

Key Responsibilities

• Conduct thorough patient histories and perform comprehensive and focused physical examinations to assess the condition of patients.
• Order, interpret, and utilize diagnostic tests to capture critical data that informs treatment decisions for chronic and acute cardiac conditions.
• Develop and implement individualized care plans that encompass health maintenance, anticipatory guidance, and appropriate medical or nursing interventions based on established protocols.
• Actively engage in follow-up care by collaborating with supervising physicians to reassess and modify treatment plans based on patient needs.
• Educate patients and caregivers on cardiac health management, emphasizing disease processes and lifestyle modifications that can improve health outcomes.
• Facilitate hospital admissions when necessary and coordinate with inpatient teams to ensure safe discharges and continuity of care post-hospitalization.
• Work collaboratively with the nursing team to triage daily patient communications, effectively addressing concerns or adjustments needed in the care plan.
• Document all patient encounters meticulously in the electronic medical record (EMR) system to maintain accurate and up-to-date patient information.
• Engage in research initiatives by reviewing and implementing findings into your clinical practice, emphasizing evidence-based care strategies.
• Support a safe, therapeutic environment for all patients and contribute to a consumer-oriented service culture.

Patient Population / Client Focus

The Gundersen Health and Wellness Clinic specifically caters to a diverse population of patients within the realm of cardiology. You will encounter a wide array of cardiac conditions, including heart failure, pulmonary arterial hypertension, and post-heart transplant care. Your expertise will also be essential in working with patients undergoing advanced therapies, including outpatient intravenous medication support and mechanical circulatory assistance. This role requires sensitivity and a proactive approach to managing chronic conditions while ensuring comprehensive education on health maintenance.

Qualifications

• A Master’s degree in Nursing with a focus on Nurse Practitioner training in adult care, ideally with experience in a cardiovascular care setting.
• Current New York State licenses as a Registered Professional Nurse and Nurse Practitioner.
• Board certification as a Nurse Practitioner.
• Hold current certifications in BCLS, ACLS, DEA, and possess an NPI.
• Adept in using electronic medical records systems and familiar with Microsoft Office applications including Word, PowerPoint, and Excel.

Preferred Experience

While a minimum of two years of experience in a hospital setting is required, candidates with experience specifically in cardiology are preferred. Familiarity with the management of patients with advanced heart failure, pulmonary arterial hypertension, and patients undergoing or being evaluated for advanced therapies will be strongly considered. Experience integrating research findings into practice is also advantageous.

Compensation and Benefits

The Nurse Practitioner role at Gundersen Health and Wellness Clinic offers a competitive salary range of $120,000 to $220,000 annually, commensurate with your experience and qualifications.
