In this full-time remote role, you will be pivotal in supporting clients uniquely affected by the complexities of hospice care, including issues such as neglect, child abuse, domestic violence, mental health challenges, and parental substance abuse. Work Environment\n

This role is designed to be fully remote, enabling you to connect with clients and manage cases from the comfort of your home or preferred workspace. As part of a supportive and flexible team environment, you will participate in regular training and supervision sessions to ensure continual professional growth. Our standard operational hours are Monday through Friday, from 8 AM to 5 PM, allowing for a balanced work-life dynamic.

Key Responsibilities\n

• Engage with clients through both telephone and video consultation to assess their needs comprehensively.

• Conduct thorough evaluations, gathering critical information regarding individual cases to formulate and implement effective treatment plans.

• Provide crisis intervention when necessary, utilizing your knowledge of crisis management techniques to stabilize urgent situations.
\n
• Maintain up-to-date and accurate documentation of client interactions and progress, ensuring compliance with healthcare regulations and standards.
\n
• Collaborate with multidisciplinary teams, referring clients to internal and external agencies when additional support is necessary.
\n
• Facilitate client engagement in beneficial activities that promote their mental and emotional health.
\n
• Contribute to a work atmosphere conducive to learning and growth by participating in individual and team meetings.
\n
• Monitor for signs of potential child abuse and take appropriate actions as mandated by state regulations.

Patient Population / Client Focus\n

Your primary focus will be on serving clients who are navigating complex and often painful experiences as they confront end-of-life issues. Many of these clients may face exacerbating factors such as mental health challenges, histories of trauma, or substance misuse, requiring a sensitive and empathetic approach. You will play a critical role in helping them process these experiences, providing guidance and therapeutic support as needed.

Qualifications\n

• A Master’s degree in Social Work, Psychology, or a related field is required.

• A current state-issued LCSW license.
\n
• Strong interpersonal skills with the ability to actively listen and engage empathetically with clients.

• A foundational understanding of mental health issues, crisis intervention techniques, and their application in a clinical setting.
\n
• Proficient use of electronic record-keeping systems and standard office software, including Microsoft Office Suite (Word, Excel, Outlook, PowerPoint).

• A valid driver's license may be required for occasional client visits or community outreach initiatives.

Preferred Experience\n

• Experience working directly with clients in hospice care settings.

• Demonstrated ability to work with diverse populations facing depression, anxiety, grief, and suicidal ideation.

• Background in social work documentation and case management.
\n

Compensation and Benefits\n

We offer a competitive annual salary ranging between $80,000 to $100,000, commensurate with experience and qualifications. In addition to a base salary, our team members have the opportunity to earn monthly productivity bonuses.
