Key Responsibilities\n

As an LCSW at CHDC, your primary responsibilities will include:

• Facilitating access to integrated healthcare services for patients, offering guidance and advocacy throughout their journey within both physical and behavioral health systems.

• Providing education and resources to empower individuals in making informed choices regarding their health, with an emphasis on wellness, disease prevention, and healthy lifestyle practices.

• Engaging actively in multidisciplinary treatment team meetings to formulate comprehensive care strategies tailored to the individual needs of patients.
\n
• Timely and accurate reporting of all crisis situations and unusual incidents, ensuring proper communication with the Medical Director and Chief Medical Officer during psychiatric and medical emergencies.

• Documenting interactions and treatments thoroughly in clinical records to maintain compliance and contribute to quality improvement initiatives.
\n
• Participating in training and development programs to enrich skills and knowledge relevant to the role, thereby enhancing the efficacy of services provided.

• Utilizing evidence-based screening tools to evaluate healthcare needs and monitor patient outcomes effectively.

• Collaborating with the Center of Excellence Team to provide specialized support to patients dealing with Opioid Use Disorder.

Patient Population / Client Focus\n

Your work will focus on supporting a diverse patient population served by the Hospice Center. This includes individuals facing life-limiting illnesses and their families, many of whom are navigating significant emotional and psychological challenges. Your expertise in behavioral health is crucial in helping these patients and their loved ones cope, understand, and make informed decisions during a vulnerable time in their lives.

Qualifications\n

The ideal candidate for this role will possess the following:

• A Master’s degree in Social Work accompanied by a valid Licensed Clinical Social Worker (LCSW) certification.

• A minimum of 2-3 years of relevant experience in a healthcare setting, demonstrating a strong foundation in clinical social work principles.

• Proven ability to work collaboratively in a multidisciplinary team environment to achieve optimal patient outcomes.

• Willingness to travel to various CHDC sites and participate in meetings, training, and outreach as required.

Preferred Experience\n

Preference will be given to candidates with experience in a Hospice or palliative care setting who are well-versed in the unique needs of patients with serious illnesses. Knowledge of integrated Primary Care Behavioral Health models and frameworks will be advantageous.
