Ontario Center, a specialized healthcare and rehabilitation provider in Ontario, NY, is seeking a compassionate and dedicated Licensed Practical Nurse to join our Healthcare & Medical Services team. In this role, you will play a key part in delivering high‑quality, person‑centered care to a diverse population of residents and patients, many with complex medical and rehabilitation needs. Working under the direction of a Registered Nurse and in collaboration with an interdisciplinary care team, you will help ensure that each individual’s care plan is followed, monitored, and continuously optimized. Responsibilities Provide direct bedside care, administer medications and treatments, and closely monitor residents’ conditions. Obtain and document vital signs, assist with wound care and dressing changes, and support respiratory and tracheostomy care as appropriate. Ensure accurate, timely documentation in the electronic medical record. Observe changes in condition, communicate concerns to the RN and providers, and help coordinate appropriate interventions that support safe and effective care. Engage in infection prevention practices, fall prevention, and other safety initiatives, while adhering to all applicable regulations and facility policies. Provide resident and family education on medications, treatment plans, and daily care routines. Assist with admissions, transfers, and discharges to contribute to smooth transitions of care. Candidate Profile Compassionate, reliable, organized, and comfortable working in a fast‑paced healthcare setting. Strong time management and prioritization skills to manage multiple residents, medications, and treatments during each shift. Confident in clear communication with residents, families, and the interdisciplinary team, maintaining professionalism even in challenging situations. Qualifications Active, unencumbered LPN license in the state of New York. Current Basic Life Support (BLS) certification. Experience in long‑term care, subacute rehab, or a similar healthcare and medical services environment is preferred, though dedicated new graduates with solid clinical training will also be considered.
