Key Responsibilities\n

• Conduct thorough psychosocial assessments to gain insights into patients' emotional, social, and financial needs, fostering a comprehensive understanding of each individual's care requirements.

• Provide both individual and group counseling addressing critical issues such as adjustment disorders, grief reactions, and coping strategies during patients’ end-of-life stage.

• Collaborate closely with interdisciplinary teams, including medical professionals and nurses, to develop integral care plans that align with each patient's treatment journey.

• Facilitate crisis intervention sessions, assuring that emotional support is readily available to patients and their families in times of distress.

• Coordinate with community organizations for seamless post-discharge services, ensuring that patients receive necessary follow-up and resources after their stay.
