A-Line Staffing is now hiring a Field Case Manager – Long Term Services & Supports (LTSS)

• Schedule: Monday–Friday, 8:00 AM–5:00 PM
• Pay Rate : - $34.13 an hour
• Benefits – active after 90 days
• Travel : 50–75% local travel; reliable transportation required in Quad City Area/Rock Island Illinois area
• Remote/Field Visits - no office to report to

Key Responsibilities

• Conduct comprehensive evaluations of members' needs, eligibility, benefits, and available resources.
• Identify high-risk factors and service needs that may affect member outcomes.
• Develop, coordinate, and implement assigned care plan activities.
• Monitor member progress and adjust interventions as appropriate.
• Coordinate with case managers, supervisors, Medical Directors, and other healthcare programs to address barriers to care.
• Participate in case conferences and multidisciplinary reviews when needed.
• Identify and escalate quality-of-care concerns through established processes.
• Advocate for appropriate services and resources based on member benefits and healthcare needs.
• Use motivational interviewing and influencing skills to encourage member engagement and healthy lifestyle changes.
• Provide members with coaching, education, information, and support to promote independent healthcare decision-making.
• Help members actively participate in healthcare decisions with their providers.
• Maintain accurate and timely documentation in accordance with regulatory, accreditation, and company requirements.

Required Qualifications

• Bachelor's degree in a human services-related field
• Minimum of 2 years of case management experience
• Valid Illinois driver's license
