Job Details Job Location: CENTRE COUNTY CSS - Pleasant Gap, PA 16826 SERVICE COORDINATOR ALLEGHENY LUTHERAN SOCIAL MINISTRIES The Community Support Services program of Allegheny Lutheran Social Ministries is actively seeking candidates for a Service Coordinator in Centre county. This position coordinates, develops, implements, monitors, and evaluates the Community Services program to ensure that the highest degree of quality service is maintained. Responsibilities Provides formal case management (i.e., evaluation of health, psychological and social needs, development of an individualized care plan for services and periodic reassessments of the resident’s situation or needs). Sets up a provider directory for use by staff and residents. Educates residents on service availability, application procedures, client rights, etc. and provides advocacy as appropriate. Monitors the ongoing service provisions of services from the community agencies and keeps case management and provider agency current with the progress of the individual. Manages the provisions of supportive services where appropriate. May educate staff on the issues related to aging in place and service coordination, to better help and assist residents. May develop case plans in coordination with community assessment services or with other professional assessment entities. Establishes linkages with all agencies and service providers in the community. This position reports to the Personal Care Home Administrator. This position works approximately 15-20 hours per week. Qualifications Bachelor’s degree in a Human Services or a related field. Two years working with geriatrics, preferably case management.
