Healthcare Consultant / Case Management Coordinator (Bilingual Spanish)
Location: Miami Beach, FL (must live in ZIP 33139, 33140, or 33141)
Schedule: Monday–Friday 8:00 AM–5:00 PM
Pay: $30/hr - $33/hr

Hiring a  Bilingual Spanish Healthcare Consultant (also known as  Case Management Coordinator ) to support members through a mix of  work-from-home case management and  in-field home visits . This is a high-impact, patient-facing role focused on helping members access care, navigate benefits, and improve health outcomes through coordinated services. If you’re a detail-oriented  case management coordinator with strong communication skills and comfort working independently, this role offers autonomy, structure, and meaningful member interaction.

Important: This is  fully remote (no office/onsite reporting) , but you will travel locally for member visits.  Mileage is reimbursed .

Responsibilities

• Conduct  telephonic and face-to-face member assessments (home visits, assisted living, nursing homes)
• Develop and implement care plans to support member wellness and safety
• Manage a caseload including supportive and medically complex members
• Coordinate services such as:
• • Prior authorizations
  • PCP and specialist/provider coordination
  • Medication review and adherence support
  • Condition management education
  • Community resources and support services
• Document interactions and care plans clearly and accurately
• Prioritize multiple cases in a fast-paced environment while meeting quality and productivity expectations

Qualifications

• Fluent bilingual Spanish/English (must read, write, and speak both)

• Comfortable with local travel (approx. 75%within the region) and visiting members in their homes/facilities
• Strong written & verbal communication, organization, and multitasking skills
• Tech-savvy and able to navigate multiple systems/tools while documenting accurately
• High School Diploma or GED required
• Comfortable working with PHI and following privacy/confidentiality standards

Training

• Remote training via Microsoft Teams for approximately 4–6 weeks
