Key Responsibilities\n

• Conduct comprehensive assessments that identify the unique mental health needs and goals of each patient, continuing to monitor progress and adapt plans as necessary.

• Communicate effectively with supervisors, colleagues, family members, and other support systems to ensure a holistic approach to each patient's care.

• Provide supervisors with an organized schedule of appointments and home visits, ensuring all necessary observations for patient monitoring are completed.
\n
• Involve yourself in coordinating home visits that follow clinical requirements, including observations and documentation.
\n
• Ensure the seamless execution of treatment plans and review the effectiveness of interventions regularly.
\n
• Participate actively in interdisciplinary team meetings to discuss treatment options and progress to enhance integrated care solutions.
\n
• Maintain accurate and timely records, including monthly and quarterly progress notes, accessible to interested parties and relevant stakeholders.
\n
• Act as a liaison between the mental health community and the public to foster support for development goals and initiatives.
\n
• Design and deliver training materials to enhance team competencies regarding program implementation and documentation processes.
\n
• Provide continual support through follow-up appointments and home visits, addressing any ongoing or emergent needs for your clients.

As a Licensed Professional Counselor at Wellbeing Counseling Center, you will work with a diverse population that includes both youth and adult clients. Qualifications\n

• Active and unrestricted licensure as a Professional Counselor (LPC) within the state of Texas.
