This role is inherently remote, allowing for flexibility while reaching clients in a hospice center. Our social workers utilize electronic health record (EHR) systems to document interactions, care plans, and progress notes. While you will primarily work from home, regular virtual meetings with the interdisciplinary team, including nurses and healthcare professionals, are crucial to coordinating patient care. Occasional visits to clients may be necessary, and on-call availability is expected to address urgent care needs.
