This position is fully remote, allowing you flexibility while engaging with a collaborative team of healthcare professionals. You will have the opportunity to conduct sessions from the comfort of your own home, coupled with tools that promote efficient communication and collaboration among team members. We utilize telehealth platforms to facilitate seamless service delivery, ensuring that our clients receive the support they need, when they need it. As part of our Hospice Center facilitation, you will contribute to holistic care, with an emphasis on emotional support for patients facing terminal illnesses.

Key Responsibilities\n

• Conduct thorough mental health assessments including psychosocial evaluations and diagnostic interviews to identify the needs and challenges faced by clients.

• Collaborate directly with clients to develop personalized treatment plans focusing on their specific goals while addressing cultural considerations.
\n
• Utilize evidence-based therapeutic interventions such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT) in a variety of settings, including individual, couple, family, and group sessions.

• Provide crisis intervention services when necessary and coordinate care with other health professionals to ensure clients in distress receive immediate and continuous support.
\n
• Maintain accurate clinical documentation in compliance with regulatory guidelines and clinic policies, including treatment plans and progress notes.
\n
• Educate clients and their families on strategies to manage mental health conditions effectively, empowering them through information and resources.
\n
• Collaborate with psychiatrists, psychologists, nurses, and case managers in a multidisciplinary team to provide coordinated and comprehensive care.

• Regularly assess treatment effectiveness and adapt plans as needed based on outcome measurements.
\n
• Keep abreast of current research and best practices in mental health to ensure the highest quality of care.

• Uphold ethical standards and confidentiality regarding all patient information.

• Master’s degree in Social Work is required.

Compensation and Benefits\n
