Work Environment\n

This role is primarily remote, allowing for flexible work arrangements that can adapt to the varying needs of clients and families. As you will be working with seniors in hospice care, expect occasional visits to clients’ residences to assess care conditions and provide face-to-face support. While the standard working hours are Monday to Friday from 8:00 AM to 4:30 PM, some evening or weekend hours may be required, based on client circumstances. The nature of the work demands flexibility, including potential on-call hours to respond to urgent client needs.

Key Responsibilities\n

• Conduct comprehensive assessments to identify emotional, mental, and social needs of seniors, encompassing life transitions such as terminal illness and bereavement.

• Develop personalized care plans collaboratively with clients and their families that align with individual goals and preferences.

• Facilitate counseling sessions, providing emotional support to clients and their families as they cope with chronic illness and end-of-life issues.

• Advocate for clients, ensuring they have access to necessary community resources and services, including healthcare, social services, and support groups.

• Work closely with an interdisciplinary team of healthcare professionals, including nurses and caregivers, to deliver coordinated care that maximizes client well-being.

• Regularly visit clients’ homes to monitor their living conditions, assess their emotional state, and address any safety concerns.

• Encourage meaningful communication among family members, helping to mediate conflicts and foster a supportive environment for the client.

• Educate clients and their families about available resources to improve social engagement and quality of life.

• Maintain meticulous documentation of client interactions, assessments, and progress notes, adhering to policies and ethical standards.

• Stay current with developments in social work practices and pertinent regulations to ensure the highest standard of care.

Patient Population / Client Focus\n

The primary focus of this role is on elderly individuals receiving hospice care, including those with complex and varying psychosocial challenges. You will engage with seniors facing life-limiting illnesses, providing them and their families with understanding, respect, and the resources necessary to navigate their unique situations.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• A current and valid state licensure as a Licensed Master Social Worker (LMSW).

• At least two years of relevant experience in social work, particularly in settings focused on elder care or hospice services.
