Key Responsibilities\n

• Guide patients in accessing valuable community resources, including support groups, housing assistance programs, and vocational rehabilitation services, tailored to their specific circumstances.

• Educate patients and families about available community resources and post-rehabilitation services, facilitating smoother transitions upon discharge.

• Actively participate in discharge planning, helping patients transition to home or other care settings effectively while maintaining follow-up care as needed.

Patient Population / Client Focus\n

You will be working with a diverse patient population encountering challenges related to addiction recovery and detoxification. Qualifications\n
