Responsibilities

• Provide accurate physical therapy assessments and develop individualized treatment plans.

• Deliver evidence-based PT interventions to improve mobility and function.

• Meet and exceed individual performance targets and contribute to pipeline goals.

• Maintain adherence to ethical standards, confidentiality, and compliance.

• Monitor patient progress and adjust plans to maximize outcomes.

• Educate patients and caregivers on exercises, safety, and home programs.

• Document all sessions accurately in electronic health records.

• Demonstrate accountability for own results within a high-visibility work environment.

• Collaborate with multidisciplinary team to coordinate care and referrals.

• Participate in ongoing professional development and pursue growth opportunities.

• Schedule and manage appointments efficiently to optimize clinic utilization.

• Contribute to a positive patient experience and high-quality service delivery.

Experience/Qualifications

• Licensed physical therapist with current PT license in applicable state.

• Demonstrated ability to conduct accurate PT assessments and create individualized treatment plans.

• Experience delivering evidence-based PT interventions to improve mobility and function.

• Ability to monitor patient progress, adjust plans, and optimize outcomes.

• Proven track record of meeting or exceeding productivity and performance targets.

• Strong commitment to ethical standards, confidentiality, and compliance.

• Strong documentation skills for electronic health records with timely session notes.

• Excellent patient and caregiver education on exercises, safety, and home programs.

• Collaboration with multidisciplinary team; coordinate care and referrals.

• Willingness to participate in ongoing professional development and growth opportunities.
