Key Highlights Weekly pay: $2,578 - $2,617 per week Guaranteed hours: 35 per week Contract duration: 13 weeks Shift: 5x8 Days (08:00–16:30) Start date: 12/01/2026 Setting: Skilled Nursing Facility, travel assignment Responsibilities Perform comprehensive PT evaluations for residents in a skilled nursing facility Develop and implement individualized treatment plans aimed at improving mobility and functional independence Provide therapy programs including therapeutic exercise, gait training, balance activities, transfers, and activities of daily living training Reassess resident progress regularly and update documentation and care plans accordingly Collaborate with an interdisciplinary team (nursing, OT, SLP) and communicate with residents and families to support discharge planning and goals Ensure compliance with professional standards and facility policies; maintain accurate records and timely charting Requirements Active physical therapy license to practice in the United States (state requirements to be met) Ability to work effectively in a skilled nursing facility environment Strong assessment, documentation, and communication skills Demonstrated ability to collaborate within an interdisciplinary team Availability for a 13-week assignment with guaranteed hours Benefits and Support Dedicated assignment support from a recruiter and ongoing assignment coordination Travel and scheduling assistance throughout the contract 13-week travel contract with guaranteed weekly hours PAY DISCLAIMER RULE Please note: Pay rates and guaranteed hours are estimates and may vary.
