This is a full-time remote position based in Victorville, California, specifically designed for a Detox Center setting. The remote work model allows for flexibility while ensuring that the essential support for our patients is maintained. Our team thrives on collaboration, utilizing online platforms for meetings, case discussions, and ongoing training to ensure high quality care, regardless of physical location. This role may occasionally include evening and weekend hours to meet patient needs.

Key Responsibilities\n

• Conduct comprehensive assessments of patients’ psychosocial needs and develop individualized treatment plans focused on their recovery.

• Engage in case management activities, assisting patients in navigating various healthcare systems, obtaining benefits, and ensuring continuity of care.

• Provide short-term counseling and crisis intervention for patients experiencing acute challenges.
\n
• Assist patients in completing essential paperwork for entitlements, supplemental insurance, and other assistance programs.
\n
• Advocate for patients with various community agencies to secure necessary resources and support services.
\n
• Maintain accurate and up-to-date documentation of patient interactions, treatment plans, and progress notes in electronic health record systems.
\n
• Coordinate transportation assistance for patients when required to ensure they can access necessary care and appointments.
\n
• Participate in team meetings and case conferences to collaborate on patient care strategies and share insights.
\n
• Deliver educational resources and referrals, particularly targeting prenatal and postpartum patients and addressing their unique needs.

• Provide HIV testing and counseling services in a supportive, confidential manner.
\n
• Monitor appointment attendance and follow up with patients who miss visits to reinforce the importance of continuity in care.

• Engage in quality improvement initiatives to enhance service delivery and patient satisfaction.

Patient Population / Client Focus\n

This position primarily focuses on the detoxification population, which includes individuals struggling with substance use disorders who require immediate support and structured care. Understanding the complexities of addiction and co-occurring mental health issues is paramount in providing effective care. Qualifications\n

• Bachelor's degree in Social Work (BSW) or related field is required; a Master's degree in Social Work (MSW) is strongly preferred.

• Minimum of two (2) years of relevant clinical experience as a social worker is required, with five (5) years being preferred.

Compensation and Benefits\n

The salary for this position ranges from USD 115,000 to 130,000 annually, commensurate with experience.
