Key Responsibilities\n

• Act as a vital liaison between clients, families, and outpatient providers, ensuring coordinated and comprehensive care.

• Engage with insurance companies for pre-certification, manage utilization review processes, and advocate for the needs of our clients.

• Participate actively in developing discharge and aftercare planning, ensuring a seamless transition for clients returning to their daily lives.

• Provide therapeutic meal support, modeling healthy relationships with food in a nurturing environment.

• Deliver safe, empathetic, and affirming care tailored to each client’s unique circumstances, considering their intersecting identities.

Patient Population / Client Focus\n

Your primary focus will be on clients undergoing detoxification and recovery from substance use disorders. This vulnerable population often struggles with complex mental health challenges, and your role will be crucial in facilitating their journey toward rehabilitation and holistic wellness. You will work closely with individuals, many of whom have diverse backgrounds and needs, to provide them with the personalized care they deserve.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• A valid LCSW license in the state of New Jersey is a must.

• Proven experience working with older adults and an understanding of the unique challenges they face.

Compensation and Benefits\n
