Luxe Home Health is seeking a compassionate and experienced Clinical Team Manager (RN) with home health experience to join our growing team.

This in-office clinical leadership and coordination role provides oversight and support for home health patients and field clinicians, ensuring high-quality, compliant, and patient-centered care. The Clinical Team Manager is responsible for coordinating patient care, supporting field clinicians, overseeing clinical documentation, and monitoring quality and regulatory compliance. This position works closely with the interdisciplinary team to promote excellent patient outcomes and consistent adherence to Medicare, Illinois, and agency requirements.

Key Responsibilities

• Serve as the primary in-office Clinical Team Manager for assigned home health patients and field clinicians.
• Coordinate care across disciplines to support patient needs and effective clinical care delivery.
• Assess, plan, implement, and evaluate individualized plans of care for home health patients.
• Provide clinical oversight, guidance, and support to field clinicians.
• Monitor patient care to ensure quality, safety, continuity, and compliance with applicable regulations.
• Ensure adherence to Medicare Conditions of Participation, Illinois regulations, and agency policies and procedures.
• Collaborate with physicians, patients, families, field clinicians, and the interdisciplinary team to address clinical needs and barriers to care.
• Review clinical documentation to ensure accuracy, completeness, timeliness, and compliance.
• Participate in quality assurance, performance improvement, and ongoing clinical process improvement initiatives.
• Identify opportunities for clinician education, improved documentation, and enhanced patient care.
• Maintain accurate, timely, and compliant clinical documentation and records.

Quality Assurance & Compliance

As part of the Clinical Team Manager's clinical oversight responsibilities:

• Conduct comprehensive clinical chart audits to ensure accuracy, completeness, and regulatory compliance.
• Review OASIS assessments and supporting documentation for quality, accuracy, consistency, and clinical support.
• Monitor compliance with Medicare Conditions of Participation, Illinois regulations, and agency policies.
• Identify documentation deficiencies and compliance concerns and provide recommendations and follow-up for correction.
• Assist with quality improvement and performance improvement initiatives.
• Support agency readiness for audits, surveys, accreditation reviews, and other regulatory requirements.
• Track, trend, and report quality findings and documentation outcomes to clinical leadership.
• Collaborate with clinicians and leadership to address recurring documentation and compliance issues.
• Support ongoing efforts to improve clinical quality, documentation practices, and patient outcomes.

Clinical Documentation Oversight

• Review Plans of Care, physician orders, visit notes, recertifications, transfers, Resumptions of Care (ROCs), and discharge documentation.
• Ensure documentation appropriately supports medical necessity, homebound status, and skilled need requirements.
• Verify timely completion and submission of required clinical documentation.
• Monitor documentation trends and identify opportunities for clinician education and process improvement.
• Promote consistency, accuracy, completeness, and continuity throughout the clinical record.
• Provide clinical guidance and feedback to field clinicians regarding documentation requirements and identified deficiencies.
• Follow up on outstanding or deficient documentation to ensure timely resolution.

Qualifications

• Active Illinois Registered Nurse (RN) license required.
• Minimum 2 years of home health nursing experience required.
• Case management or care coordination experience in a home health setting.
• Strong knowledge of home health regulations, Medicare requirements, and clinical documentation standards.
• Experience with OASIS and home health clinical documentation.
• Strong understanding of Plans of Care, physician orders, medical necessity, homebound status, and skilled service requirements.
• Strong clinical judgment and attention to detail.
• Excellent communication, organizational, leadership, and problem-solving skills.
• Ability to effectively support and collaborate with field clinicians and the interdisciplinary team.
• Ability to identify clinical, documentation, and compliance concerns and work collaboratively toward resolution.
